Passport
Before your appointment, you will need to bring the following...

Completed DS11 Passport Application forms (in BLACK ink) - found here.

2x2 inch Passport photo (can be taken at Walgreens, CVS, AAA).

Photo Identification (Driver's License, Military ID, etc.) If the applicant is a minor*, this would be the photo identification of their parents/guardians.

Proof of US Citizenship (long-form birth certificate with raised seal, naturalization certificate with raised seal, undamaged expired passport). For a definition of a long-form birth certificate, please click the link here. If you are a Connecticut resident, you can apply for a copy to be mailed to you by following the link here.

Check or money order made out to the US Department of State ($130 per adult, $100 for minors under the age of 16) for a fee breakdown, follow the link provided here.

Check, Cash, or Credit Card Payment to Hall Memorial Library ($35 processing fee, which is mentioned in the link provided above).

*Note - If the appointment is for a minor (under the age of 16), BOTH parents/guardians and the minor MUST be present for the passport appointment. If one parent/guardian cannot attend, the missing parent/guardian must complete a DS-3053 form, have it notarized, and give it to the parent/guardian who can attend the appointment.

Notary
The Library has a Notary Public available to notarize your documents. Please call in advance to schedule an appointment, and ask for Sue.
Documents MUST be signed in the presence of the Notary. DO NOT sign them beforehand.
Fees: One to two signatures/stamps are free. Three or four is $5.00. Five or more is $10.00.
